Philippines Passport Renewal Requirements

To renew your Philippine passport, you need your current passport, an appointment confirmation and supporting documents.

Basic Requirements for Simple Adult Renewal

  • Confirmed online appointment – Printed appointment packet (sent via email)
  • Accomplished application form – taken from the online portal
  • Current electronic passport – Both original and a photocopy of the data page (the page with your photo)
  • Personal Appearance – for biometrics and photo capture

Additional Requirements (When Needed)

  • PSA marriage certificate or report of marriage – for change of name
  • PSA death certificate of spouse or PSA marriage certificate with annotation of annulment/divorce – for changing back to maiden name
